Athletic Forms

   In order to participate in any of North's sports and athletic clubs, several forms need to be turned into the business office. You can download all the forms here. Please be aware that you need to turn the completed and signed paper copy into the business office. You cannot e-mail the forms to school. (Forms are in PDF format)

1. Athletics Form (PDF)   Includes Athletic Permit,  Code of Conduct, WIAA Eligibility, Concussion Information,
      New Athletic Permit Form   Use this form instead of then one on the link on the line above. Use the link above for the other forms.

         All Parents/Students need to complete & sign both sides:
                    - Top portion on pages 1-2
                    - AASD Athletic Code of Conduct, page 2
                    - WIAA HS Athletic Eligibility Information, page 2

         If Current Year Physical:
                  - Physical provider (physician) must clear the athlete, sign, and date area provided on page 1.
                  - Parent completes Athletic Permit & Liability Waiver on page 1

         If Alternate Year Physical:
                 - Parent completes Physical Date & Alternate Year-Athletic Liability Waiver on page 1 and signs waiver.

Travel Release Form  If a student is unable to take school-provided transportation to or from an event, a Travel Release form must be turned in to the Business Office 24 hours prior to the event.
