Connected Community Wellness Screen

The Appleton Area School District partners with the Connected Community Wellness Screen, offering free, confidential emotional wellness screenings to students.

  • Starting from 2019-20, all third-grade students were invited to participate, aiming for 100% involvement.
  • Since 2017-18, ninth and eleventh-grade students have received free, confidential screenings at their schools.
  • Since 2015-16, every ninth-grader in the district has had the opportunity to participate.

Screenings will take place in the fall and spring semester at each high school. 

What is Connected Community Wellness Screen?

Connected Community Wellness Screen is an emotional wellness screening program for youth, which uses an evidenced based public domain screening tool called the Pediatric Symptom Checklist-Youth Self-Report (PSC-Y), which is also endorsed by the Wisconsin Department of Public Instruction (DPI).  

The PSC-Y is a free, confidential and voluntary mental health checkup for teens. Early detection of and intervention in emotional wellness concerns, as well as getting connected with mental health services, can save lives.


How does it work?

First, families provide consent for the screening. Then the teens themselves agree to participate in the two-step screening process: 

  • Teens complete a 10 minute computerized questionnaire exploring vision, hearing and dental problems, symptoms of depression and anxiety, suicidal thinking and behavior, attentional problems, disruptive behavior, and use of drugs and alcohol.
     
  • Teens meet with a member of the Wellness Screen staff to discuss the screening process. When a screen indicates concern, families are notified and assisted in connecting with a community resource for further evaluation.
