IMMUNIZATION OF STUDENTS
Immunization Policy: State law requires that any student admitted to an elementary, middle, or high school shall, within 30 days after admittance, furnish the school with one of the following:
- Written evidence that the student meets the minimum immunization requirement for that student’s grade and is on schedule for the remainder of the basic and recall immunization series for diseases identified in this statute;
- A completed immunization waiver form provided by the parent/guardian or adult student to the school objecting to the immunization for reasons of health, religion, or personal conviction.
Any student enrolled in the Appleton Area School District (AASD) who is not in compliance with requirements outlined within the legal timelines, will be reported to the District designee.
It shall be the responsibility of the principal, along with designated building support staff, working in consultation with the school nurse to ensure that students entering the AASD have had the required immunizations.

IMMUNIZATION OF STUDENTS
Procedures
- As a part of the enrollment process for each first time admission, the school shall obtain from the parent/guardian or adult student an accurate and complete immunization history indicating that the student meets the minimum immunization requirements or permission of the parent/guardian or adult student to be immunized:
- By a private physician specified by the parent and/or
- By the local public health agency.
Immunization requirements may be waived if the parent/guardian or adult student submits a written statement to the school objecting for the reasons of health, religion or personal conviction.
- The immunization history shall be in a format prescribed by the Department of Health and Family Services.
- If at that time the history indicates that the student does not meet the minimum immunization requirements [based on Advisory Committee on Immunization Practices (ACIP) recommendations and described in the compliance form issued by the Department of Health and Family Services] under State law, and the parent/guardian or adult student chooses not to grant permission to be immunized, the school shall obtain from the parent/guardian or adult student one of the compliance alternatives within 30 days.
Immunization requirements may be waived if the parent/guardian or adult student submits a written statement to the school objecting for the reasons of health, religion or personal conviction. - Within 40 days after admission for any particular student, the school shall submit a report to the local public health agency indicating the number of students who do not meet the minimum immunization requirements. The initial report shall be in a format prescribed by the Department of Health and Family Services and will include statistical information concerning the compliance alternative selected for all first time admissions.
- The school shall maintain on file, the immunization histories of students entering school for the first time and the compliance alternatives selected by the parent/guardian or adult student who do not meet the minimum immunization requirements.
- The school shall update immunization histories with information provided by the local public health agency, parents/guardians, adult students or private physicians.
- The immunization history or record of any student who transfers from one school to another shall be forwarded to the new school in compliance with legal requirements.
- If at that time the history indicates that the student does not meet the minimum immunization requirements [based on Advisory Committee on Immunization Practices (ACIP) recommendations and described in the compliance form issued by the Department of Health and Family Services] under State law, and the parent/guardian or adult student chooses not to grant permission to be immunized, the school shall obtain from the parent/guardian or adult student one of the compliance alternatives within 30 days.
