Wisconsin Immunization Law

The Wisconsin Immunization Law requires that all students through grade 12 meet a minimum number of required immunizations prior to school attendance. These requirements can be waived only for health, religious or personal conviction reasons. Please provide a copy of your child’s immunization record to the school office at the start of the school year.

Immunization registries are an integral tool for assuring that children and adults receive immunizations according to recommended schedules, and can prevent over-immunizing.

Families and individuals can use this screen to view and print their immunizations. First Name, Last Name, and Birth Date are required. Available in English, Spanish and Hmong.
