Eligibility
Employees enrolled in the Appleton Area School District Health Insurance Plan (along with their covered spouses, newborn+ (District Health Employer Clinic), children 1 and up (Connecting Care Clinic), dependent children until age 26, and retirees until age 65) can utilize the services provided by the employee health clinics.

ThedaCare has had a longstanding partnership with the Appleton Area School District serving employees and their families at the Connecting Care Clinic. The clinic provides primary care services which include, annual physicals, chronic condition management, medication management, acute care, lifestyle medicine, and lab services.
The clinic also has onsite Physical Therapy and a Health Coach. The team strives to enhance and create a more comprehensive approach to care, with the goal to meet employees and their families where they are at by being more proactive with their health and wellness goals.

Appleton Area School District and Menasha Joint School District have joined together to offer this valuable primary care service to members on their health plans. This partnership allows District Health to offer expanded hours and services, and more convenience for you and your family.
District Health provides staff with a comprehensive care option including MD services, a close connection with the AASD Health Plan Navigator for easy access to potential no copay referral options, and a dispensary for onsite prescriptions.
Services at both clinics are provided as a no cost option to those covered on the District's health insurance plan.
