  Sports Form
The Sports Form is a two-page form for all needed signatures. It must be on file prior to participation.

If your athlete needs a new physical, please also complete the Physical History Form (below) prior to your doctor visit as a convenience to your physician. Be sure to get your physician's signature on the Sports Form.​
  Physical History Form (Note: This form is to be filled out by the patient and parent prior to seeing the physician. The physician should keep this form in the chart.)

A physical is good for two years if it is taken after April 1st. The athlete must fill out the alternate form in their second year, which is part of the Sports Form.

Please fill out this history form and bring it to your doctor for your physical, but ask the doctor to fill out your Sports Form to clear you for participation (above).​ In second year, just fill out the Sports Form (above).
   Concussion Parent & Athlete Agreement Form As a Parent and as an Athlete it is important to recognize the signs, symptoms, and behaviors of concussions. By signing this form you are stating that you understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury.

  WIAA Eligibility Bulletin It is the responsibility of the athlete and parent to read the information contained in the WIAA Eligibility Bulletin and then sign in the appropriate portion of the Sports Form document.​

  Travel Release Form A new travel release form needs to be filled out for each event that your son or daughter does not travel with a scheduled team bus. We encourage athletes to travel on all school buses, when provided, as this is an important part of the team building experience.​
